In this tutorial, I show you how to create an online submission form using Google Docs, and also how to email that form out to others.<\/p>\n

Creating the form<\/h3>\n

  1. First we're going to create an online form using Google Docs, so log into your Google Docs account:\u00a0http:\/\/google.com\/docs<\/a>.<\/li>\n
  2. Click the 'CREATE' button.<\/li>\n
  3. Select 'Form'.<\/li>\n
  4. Enter in 'Online Form' in the field Titled 'Untitled form'.<\/li>\n
  5. In the 'Question Title' field type 'What is your name?'.<\/li>\n
  6. In the 'Help Test' field type 'Enter your first and last name.\"<\/li>\n
  7. Set the 'Question Type' to 'Text'.<\/li>\n
  8. Check the box next to 'Make this a required question.'<\/li>\n
  9. Click the 'Done' button.<\/li>\n
  10. Mouse hover over the 'Sample Question 2' field and click the 'pencil' edit icon button.<\/li>\n
  11. In the 'Question Title' field enter 'What is your email address'.<\/li>\n
  12. In the 'Help Text' field enter 'Enter you email address'.<\/li>\n
  13. Set the 'Question type' to 'Text'.<\/li>\n
  14. Check the box next to 'Make this a required question'.<\/li>\n
  15. Click the 'Done' button.<\/li>\n
  16. Click the 'Add item' button at the top of the screen.<\/li>\n
  17. Select 'Paragraph text'.<\/li>\n
  18. In the 'Question Title' field enter 'What is your message?'.<\/li>\n
  19. In the 'Help Text' field enter 'Enter your message here'.<\/li>\n
  20. Set the 'Questions type' to 'Paragraph text'.<\/li>\n
  21. Check the box next to 'Make this a required question'.<\/li>\n
  22. Click the 'Done' button.
    \nYou can add any form variables you need for your form this way.<\/li>\n
  23. Click 'More actions' at the top of the screen.<\/li>\n
  24. Click 'Edit confirmation'.<\/li>\n
  25. Replace the default confirmation message text in the field with something more suitable for your form function.<\/li>\n
  26. Click the 'Save' button.<\/li>\n
  27. Click the 'See responses' button.<\/li>\n
  28. Click 'Spreadsheet'. When someone sends their information, it will be saved to this spreadsheet so keep that in mind.<\/li>\n
  29. Click on 'Tools'.<\/li>\n
  30. Click on 'Notification rules'.<\/li>\n
  31. Check the box 'A user submits a form'.<\/li>\n
  32. Check the box 'Email - right away'. Now when someone submits the form, you will be notified through email.<\/li>\n
  33. Click the 'Save' button.<\/li>\n
  34. Click the 'Done' button.<\/li>\n
  35. Click on 'Form'.<\/li>\n
  36. Click on 'Edit form'.<\/li>\n
  37. Click the 'Email this form' button. Email the form to whoever you like.<\/li>\n<\/ol>\n
